THE PROMOTION OF SOCIAL INCLUSION AMONG ELDERLY, VULNERABLE AND HOUSEBOUND PERSONS WHO ARE EXCLUDED FROM SOCIETY BECAUSE THEY ARE HOUSEBOUND, OR WHO ARE UNABLE TO DRIVE OR EASILY ACCESS PUBLIC TRANSPORT, BY PROVISION OF: • A BEFRIENDING SERVICE WHICH WILL RELIEVE LONELINESS, PROVIDE SOCIAL CONTACT ON A REGULAR BASIS WHERE REGULAR SOCIAL CONTACT MAY NOT EXIST;• ASSISTANCE TO HELP THEM INTEGRATE INTO SOCIETY; • OUTINGS AND SOCIAL TRIPS INCLUDING TRANSPORT TO AND FROM HOME;• LIAISON WITH EXTERNAL SERVICE PROVIDERS TO ENSURE BENEFICIARIES CAN ACCESS THE SERVICES AND FACILITIES THEY REQUIRE TO PROMOTE HEALTH AND WELLBEING AND ENABLE THEM TO PARTICIPATE FULLY IN SOCIETY.
