TO ASSIST PARENTS OF, OR OTHER PERSONS LOOKING AFTER SICK, ILL OR DISABLED CHILDREN AND YOUNG PERSONS UNDER THE AGE OF 18 YEARS IN MEETING EXPENSES AS A CONSEQUENCE OF THE SICKNESS, ILLNESS OR DISABILITY WHICH MAY NOT OTHERWISE BE MET FROM ANY OTHER STATUTORY OR CHARITABLE SOURCE. ANY ASSISTANCE GIVEN SHALL BE AT THE DISCRETION OF THE TRUSTEES. IT IS EXPECTED THAT NORMALLY ALL REQUESTS WILL BE MADE BY MEMBERS OF THE SALTASH PRIMARY HEALTH CARE TEAMS. WHERE A REQUEST IS MADE OTHERWISE THAN VIA A SALTASH HEALTH CARE PROFESSIONAL THEN SUPPORTING EVIDENCE OF THE HEALTH CONDITION MUST BE PROVIDED.
